If you don’t see an appointment time that works for you please use the “Contact” button, so we can offer you other times or add you to a waitlist.
Please include what you would like to focus on in therapy and the name of your insurance.
After you have been scheduled our billing team will verify your insurance. To streamline the onboarding process please include the below information in your appointment request or when you contact us.
For OHP (Medicaid), Medicare, Pacific Source, Providence, and Moda insurance include the name of your insurance and your member ID.
For all other insurance companies include the following information: does your plan offer out-of-network coverage, do you need a pre-authorization, do you need to pay a copay or coinsurance, and is the coverage subject to a deductible?
