• Your insurance information

After you have been scheduled our billing team will contact you to insure we have all the insurance and billing information we need. To streamline the onboarding process please have the below information ready.

• For OHP, Pacific Source, Providence, and Moda insurance include the name of your insurance and your member ID.

• For all other insurance companies include the following information: does your plan offer out-of-network coverage, do you need a pre-authorization, do you need to pay a copay or coinsurance, and is the coverage subject to a deductible?
